• Consent for Nonsecure Communication by Email

  • Although we offer a patient portal for the secure transmission of electronic health information, many patients prefer the ease of unsecured email.

We are willing to send and receive health information by unsecured email, but it is important for you to know that email is not considered to be a confidential means of communication and that it is possible for a third party to intercept and eavesdrop on those messages. While Etheridge Psychology’s electronic systems are secured, information is no longer protected by our systems once an email is sent. Information may be vulnerable while in transit and after it is received by your email server. The kinds of parties that may intercept these messages include, but are not limited to:

    • People in your home or other environments who can access your device
    • Your employer, if you use your work email to communicate with us
    • Third parties such as server administrators and others who monitor internet traffic
    • People who may “hack” email or computer

If, after reading these risks, you have decided you prefer to use our secure patient portal, please do not complete this form and contact us at (919) 600-4906.
