We are so excited you have chosen Interlachen Pediatrics to care for your child!  This series of forms have been designed to gather basic information about you and your child. We have also provided some basic information about our practice. Our practice uses an electronic medical record and therefore collects data electronically. All information is gathered and stored on HIPAA-compliant servers. We look forward to seeing you and your child in our office!
