You agree to follow any and all of Sunrise Physical Therapy Services, Inc. (SPTS) protocols. The use of the office is for utilizing the equipment and you are not currently receiving physical therapy under the direction of a SPTS physical therapist. I understand I am using the equipment at SPTS at my own the risk and release SPTS of any and all liability.
