Dear Client:
This letter is to advise you of Beautiful Minds Medical’s grievance procedure. If you have a concern or complaint please immediately inform the staff you are working with so we can provide immediate attention to your concern. If your issue is still unresolved, please feel free to ask for a supervisor. Most complaints are resolved immediately. In the event you would like to file a grievance, which you may do at any time, ask any staff member for a Grievance Form.
Once a Grievance Form has been filed you will receive an acknowledgement that it has been received within 24 hours by the Patient Ombudsman or their designee. Your complaint will be investigated by the Ombudsman and you will receive a written response within 10 working days. The written response will inform you of how your complaint was investigated, who conducted or was involved in the investigation, and the outcome of your grievance.
