Payment Options:
We are proud to be a team whose primary mission is to deliver the finest and most comprehensive periodontal services available today. It is very important to us that finances do not become an obstacle to obtaining your treatment. We have developed a financial policy that allows multiple options for you to help you meet your needs.
Our goal is to provide you with optimal care based on your individual needs. To ensure a positive experience, we ask our patients to take a moment to review our financial policies. Payment is expected at the time services are rendered. Payment for surgical procedures are required in advance of the day of treatment (usually 1 week unless other arrangements have been made). This policy is in place, as many of our patients are sedated for surgery, and we legally cannot address financial matters during that time. Our practice accepts Cash, Checks, and Credit Cards (MasterCard, Visa, Discover & American Express) for payment. Should you desire an extended payment plan, we offer No Interest Payment Plans financed through Care Credit. If you choose to cancel treatment and payment was made on a credit card or with a financing company, the refund will be issued to the card payment was made with minus any credit card processing fees.
Insurance:
As a periodontal practice our relationship and responsibility is to you, the patient, and not to the insurance company. Unless other arrangements are made in advance, payment is due in full at our office. As a courtesy, we will electronically file all of the necessary forms, x-rays, and charting for you with your carrier. In some instances, we may accept direct payment from your insurance company. Should we accept direct payment, we will estimate your deductible and the portion not covered by your insurance. This amount is due from you in advance of your treatment. Our estimates may be different than your insurance company's calculation; therefore, the amount due our office may be adjusted accordingly. You may find that our fees may be different from the insurance company's schedule of "allowable" or "UCR" fees. If you have any questions about "UCR" fees, please feel free to ask.
All services rendered are charged directly to the patient and the patient is ultimately responsible for the account regardless of insurance coverage. Any insurance claims denied, altered, or remaining unpaid after 45 days from the date of service will automatically become the responsibility of the patient. We will gladly discuss and answer any questions concerning your proposed treatment plan and insurance coverage. The insurance you have is a legal contract between YOU, your employer, and YOUR insurance carrier. Our office is not and cannot be a part of that legal contract. In addition, not all services are a covered benefit in an insurance contract. Insurance companies do NOT cover many periodontal or implant related procedures. Dental benefit plans will never pay for the completion of your dental care, it is only meant to assist you.
Appointments / Cancellations:
We reserve time specifically for you. Our office strives to run on time so that you do not experience long waits in our reception area. Please be on time for your visit with us, so that we can continue to meet this goal. Should you need to reschedule, please allow at least 2 business days notice for a non-surgical appointment and 7 business days for a surgical appointment. Cancellation fees may be added to your account if proper notice is not given.
