Our Medication Ajustment Service means treatment prescribed by our physicians will be monitored for up to about 6 months after the medication has been started. Our Medical Doctors on this service will collaborate in your care to help find the right medication and dosage before your care is returned to your own doctor in the community.
AGREEMENT FOR ACCEPTING OUR REFERRAL AND MEDICATION ADJUSTMENT SERVICES
- Once this initial adjustment is underway, usually within 6 months, your care will be returned to your doctor or your child's primary care physician in the community. This is usually the physician who referred you or your child to our clinic. Our Medical Doctors on the Medication Adjustment Team at Possibilities do not provide general primary patient care or ongoing medication refills and support.
- Our Medical Doctors Services are offered through hybrid in-person and secure video sessions. Therefore, you will need to keep track of measurements like heart rate, height and weight at home. For blood pressure, you’ll need a home blood pressure monitor. Recommended BP monitors can be found at Hypertension Canada. Cuff sizes on blood pressure monitors are different for different ages. So if this service is for your child, be sure to purchase the appropriate cuff size.
- In some cases, our Medical Doctors will determine that it may be unsafe to prescribe medications without an in-person physical exam. Your own Family Physician or Paediatrian will need to provide this service while collaborating with us.
