Financial Assistance Request Form

New Hope Equine Assisted Therapy provides Equine Assisted Therapeutic Horseback Riding in the form of weekly one-hour lessons.  We offer financial assistance to those with incomes under $50,000 annually based on available funds for scholarships.

The rider is expected to attend classes regularly.  Financial assistance will be revoked for riders who have more than 3 unexcused absences or 3 incidences of not calling to cancel when unable to ride. 

You must include a copy of 2 recent paystubs for each working parent/guardian before this application will be considered. Alternate forms of income may be provided in lieu of paystubs such as Award letters for any form of government assistance (including, but not limited to: Unemployment Compensation, Workman’s Compensation, Food Stamps/SNAP, Disability, Social Security, Medicaid, Medicare, Sooner Care, Heartland and TANF).

All financial assistance is granted based on the New Hope Equine Assisted Therapy policy established by the Board.
