Welcome to the Possibilities Clinic! You and your child have an in-person appointment with our Clinician.
What will happen at the appointment?
For certain assessments, it is important for our Clinician to meet with your child in-person so this in-person visit is being offered. This appointment is a chance for our Clinician to take down all relevant information from you (parent/caregiver) and to interact with your child to get to know your child better and to observe behaviours.
What COVID-19 precautions will be in place?
- Our Clinician will be wearing a mask and also a face shield.
- The office will be cleaned between each patient visit.
What are my COVID-19 requirements?
- Only ONE parent will be allowed in the office.
- Parent must wear a mask.
- If your child is able to wear a mask comfortably, then please have your child wear a mask. All children/teens ages 6 and older must wear a mask.
- Only the child who is the patient will be allowed into the clinic. Siblings will not be allowed to attend the appointment.
- Children are not allowed to bring food, toys, or electronics into the office
- It is fine to bring water for you and your child as long as it is in a water bottle.
Please note that if you arrive without the parent/patient requirements in place, then you will not be able to be seen and the appointment will be considered a late notice cancelation. Late notice cancelation fees may apply.
I acknowledge the contagious nature of the Coronavirus/COVID-19 and that many public health authorities still recommend practising social distancing.
I further acknowledge that Possibilities Clinic has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19.
I further acknowledge that Possibilities Clinic cannot guarantee that I will not become infected with the Coronavirus/Covid-19.
I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, staff, and other clients and their families.
I voluntarily seek services provided by Possibilities Clinic and acknowledge that I am increasing my risk of exposure to the Coronavirus/COVID-19.
I acknowledge that I must comply with all set procedures to reduce the spread while attending my appointment.
I attest that:
* I am not experiencing any symptoms of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.
* I have not traveled internationally within the last 14 days.
* I have not traveled to a highly impacted area, including the USA in the last 14 days.
* I do not believe I have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19.
* I have not been diagnosed with Coronavirus/Covid-19 and not yet cleared as non contagious by provincial or local public health authorities.
* I am following all recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID-19.
I hereby release and agree to hold Possibilities Clinic harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the clinic, or that may otherwise arise in any way in connection with any services received from the Possibilities Clinic.
I understand that this release discharges Possibilities Clinic from any liability or claim that I, my heirs, or any personal representatives may have against the clinic with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from Possibilities Clinic. This liability waiver and release extends to the clinic together with all owners, partners, and employees.
