PERSONAL RESPONSIBILITY AND AGREEMENT FORM
2020-2021
* I agree to care for the Chromebook or iPAD, case, charging cube and charging cable assigned to me.
By signing this agreement, I understand that failure to return these items at the
completion of the 2020-2021 school year or before my withdrawal from the
Morgan County School District to attend another school will result in the
charges listed below. All necessary means will be used in collecting devices from students not returning in District-owned equipment to include law enforcement assistance and reporting to collections.
Further, if any of the below equipment is damaged upon return due to neglect or
beyond normal usage the following charges will be assessed to the student:
Lost or Unusable Chromebook or iPad = $300
Lost or Unusable Chromebook or iPad Case = $30
Lost or Unusable Charging Cube = $10
Lost or Unusable Charging Cable = $10
DIGITAL FORM SUBMISSION IS ACCEPTANCE OF DEVICE AGREEMENT TERMS, POLICY 5137 and ACCEPTABLE UE POLICY
