Patient Information and Acknowledgement: I authorize Mobile Medical Health LLC, and its associates, to perform Covid testing. We may share your demographic information with our location Stone Ridge Pharmacy.
We offer three types of testing, rapid antigen nasal swab test, PCR test and rapid fingerstick antibody testing. Both antigen and PCR tests detects human SARS COV 2 antigen in response to the virus presence in the human body. It is performed using nasal swab and collected from individuals suspected of COVID -19 by a healthcare provider. The antibody test is performed using a fingerstick method and determines possible recovery from the Covid virus.
Positive rapid antigent and PCR results will be sent to Virginia Department of Health as required by law. A positive test result can indicate current infection or exposure to Covid-19. A negative test may indicate an immune response has not yet been detected, and if symptomatic, the patient may need repeat testing. False positive / false negative results are possible, we offer confirmatory pcr testing but it is not required.
The symptoms of Covid-19 include and are not limited to: cough, shortness of breath, loss of taste/ smell, difficulty breathing, rash, nausea/ vomiting/ diarrhea, muscle pain/ fever, chills, headache, sore throat.
The median incubation period is 5 days, but can range from 2-14 days. If results are positive, the patient poses a risk to public health and must quarantine for 14 days.
Potential Risks and Benefits of the test:
Possible discomfort or other complications during the sample collection
Possible incorrect result - a negative result does not preclude Sars- Covid 19 infection. Possible false positive results.
The FDA cautions patients against using the results from any covid test as an indication that they can stop taking steps to protect themselves and others, such as stopping social distancing or discontinuing wearing masks. We At Mobile medical Health are trying to mitigate the impact COVID-19 has on our community by performing COVID-19 testing. If you continue to have symptoms, Despite an initial negative test, repeat testing is recommended. If you test negative for COVID-19 you probably were not infected at the time your sample was collected. However that does not mean you will not get sick. The test result only means you did not have detectable COVID-19 at the time of testing.
A positive test reult can indicate the patient has detected antibodies to the SARS-Covid 19 and potentially been exposed to Covid-19. The test is used to help limit the spread of Covid-19 to your community. The department of health will contact you if you are positive for Covid-19 to follow up and determine contacts and or quarantine needs. False positive can occur, and therefore PCR testing is recommended to confirm.
I confirm that testing is medically necessary for the diagnosis of a disease, illness or infection. The results will be used as medical management/ treatment decisions.
By signing this document, I agree that I will not hold Mobile Medical Health LLC or its affiliates responsible for extending waiting periods, unexpected extended work or school leave, or travel disruptions and changes due to delayed, false positive or false negative testing results. I understand that should I require a result by a specific date, it is my responsiblity to plan accordingly to receive them on time. It is also understood travel locations may change their testing requirements, and it is your responsibility to determine your exact needs. We recommend using rapid antigen testing for patients with Covid-like symptoms for 2-3 days. We recommend using pcr testing for asymptomatic patients, travel requirements, possible Covid-19 exposure and confirming rapid positive and negative testing. PCR testing is currently the most sensitive and accurate of all Covid testing.
