In this section, we ask about your medication history. Insurance will often cover these treatments if we have DETAILED History, if you can not remember the history we obtain it from past prescribers and even have you request pharmacy records. TMS is FDA cleared after one medication fails, but most insurance companies require more medication failures and therapy failures (live or remote options). Esketamine is indicated similarly. We recommended TMS before Ketamine in most instances because when Esketamine works it becomes a maintenance treatment (meaning that you have to return for further treatments regularly to keep you well). TMS has good durability. Most patients who get well, stay well. In our office, approximately 15% of patients don't respond to TMS (there is not an association between the length of time which a patient is sick nor the severity of mood). When treatments fail we look to other medications, and different therapies, like DBT, and even ECT, and VNS treatments. THERE IS ALWAYS HOPE.
