Below are the expectations and guidelines for the Suboxone program. Your continued participation in the program will be based on your adherence to these expectations and guidelines.
Program Expectations of Patient:
- Keep my scheduled appointments and/or communicate with my care team for schedule changes
- Conduct myself in a courteous manner with all staff and fellow patients
- Discuss all prescription, over the counter medications, and herbal supplements I am taking
- Openly discuss triggers and participate in relapse prevention planning
- Openly discuss relapses, as applicable
- Meet with the Licensed Alcohol and Drug Counselor to discuss a counseling plan
- Take Suboxone as prescribed including appropriate dosage, directions, and route
- Be working towards sobriety
Program Expectations of NHS:
- We will engage in open communication in a courteous manner and we will listen to any concerns
- We will return all phone calls within one business day
- We will work with you through any set backs or concerns, no one will be dropped from the program without a discussion and, if indicated, may be referred to another program
Program Guidelines:
- Lost medications will not be replaced
- Continued use of illicit substances will result in referral to treatment and may limit access to Suboxone
- All Suboxone prescriptions, including refills, require a clinic visit
- You will be contacted for random urine drug screens which may be supervised by an NHS staff member of your identified gender
- If you regularly take prescribed benzodiazepines you will be required to taper and discontinue this medication, in collaboration with your medical provider
Reasons for Termination from Suboxone Clinic:
- Selling, sharing, or giving Suboxone medication to any other person
- If your urine drug screen indicates that you are not taking Suboxone as prescribed
- Continued use of unprescribed benzodiazepines
- Obtaining Suboxone from more than one provider (outside of NHS)
- Any attempts to falsify sample during urine collection
- Failure to meet minimum contact requirements with your care team
This form must be signed to acknowledge that you have reviewed this information with a Suboxone Team Member and you are ready to enroll in the program.
