Questions and answers are to help you better understand our innovative direct Primary Healthcare Plan and what our staff has to offer. You do not need to be a member of the GloFusion direct primary care Plan to visit GloFusion Clinic.
What is Direct Primary Care?
Direct Primary Care (DPC) is a model of healthcare that utilizes a monthly membership model to allow patients greater access and a more personal relationship with their provider. By cutting out the insurance companies, we are able to spend more time ensuring each patient receives individualized care that best suits the patient. Do you see children?
Yes, we see children of all ages.
Do you take "walk-ins"?
Yes, we accept "walk-ins, no appointment is necessary to visit GloFusion clinic, however appointment is preferred. You may visit the clinic during its normal hours of operation.
Is this health insurance?
No, we are not an insurance company, nor are we offering insurance. We are simply offering an affordable, alternative payment plan to access basic primary and urgent care via membership into our Primary Healthcare Plan. This will not satisfy the requirements for insurance under the Affordable Healthcare Act.
How do I pay my fees?
Membership fees are automatically billed to your credit card or debit card monthly. Any extra chargers are due at the time of service. We accept cash or debit/credit cards (Visa, MasterCard, Discover, and Health Savings Account cards with the Visa/Mastercard logo).
What if I don't use your services? Do I still have to pay for my membership?
Yes, membership into our Direct Primary Healthcare Plan is a one-year contract. We are offering these services at a discounted rate and allowing payment to be spread over one year's time.
What if I go to another doctor or the emergency room? Will you pay my bill?
No, your annual membership fee ensures you access to care at one of our participating Providers at GloFusion Clinic. We will assist in referring you to other healthcare providers, but the expense of their healthcare is not included by this agreement
Is this a one-time fee?
No, the membership agreement is for one year and meant to be renewed annually. Cancellation of the membership must be received in writing 60 days before the annual renewal date.
What if I need hospitalization or surgery?
We will advise you if we feel hospitalization is necessary, however, the Direct Primary care Plan does not cover the cost of hospitalization. Our membership plan covers services as outlined in the Membership Agreement.
Will I receive a discount if I don't have my annual physical exam?
No, your annual membership cost is $468 and is non-refundable. We are allowing you an alternative payment plan of $39.00 per month plus a one-time sign-up fee of $59.
Is the cost of prescriptions covered or discounted through my membership fee?
No, prescription drugs are not included with your membership.

You may be seen at GloFusion Clinic, Griffin location during normal hours of operation. Visit www.glofusionhealth.com for our hours of operation.
I woke up this morning with a red crusty eye. It hurts a little. Can I be seen?
Yes, visit GloFusion clinic during normal operating hours. This visit will be covered at no extra cost.
My throat has been hurting for a couple of days and is not getting better. Can I be seen?
Yes, this visit will be covered.
My child has a fever and is irritable. Can I come in?
Your child's visit will be covered if your child is a member of our GloFusion Direct Primary care Plan.
I have chronic hypertension. Can I purchase a membership in the Direct Primary care Plan? Yes, you are eligible for membership even if you have a pre-existing condition.
I fell and hurt my ankle. I believe something could be seriously wrong. Can I come in? Yes. This visit will be covered by your membership, however if the Provider recommends that you need an X-ray, this will be done outside of our office and will not be covered by the membership in our Plan.
I have a burning sensation when I use the restroom. Is it covered?
Yes, your visit will be covered but any recommended lab work would be an extra cost. With 50% discount.
I broke a glass and cut my hand. My hand is bleeding and I can't make it stop. Can I use this with the membership ?
Yes, visit GloFusion clinic during normal operating hours and you will be treated as deemed necessary. If stitches are required, there will be an extra charge which is 50 % discounted.
It is Spring and my allergies are acting up. Can I come and get a prescription to make me feel better?
Yes, you can be seen, and a prescription will be written if it is medically necessary. Remember you have unlimited visits with your membership.
I have terrible migraine headaches. I would like to take something so I might be able to prevent them. Can I be seen?
Yes, Remember you have unlimited visits with your membership.
I have been out working in the yard today. This afternoon my arm has a rash that is burning and itching. Can I be seen?
Yes, you may be seen and if deemed medically necessary, prescriptions will be written and included under our membership plan. The cost of filling your prescription will not be included with membership.
My child is getting ready to start volleyball at school. We need to have a sports physical done. Can we make an appointment?
Yes, if your child is a member in our Plan, this visit will be covered.

Membership Agreement
1) PROGRAM: This Membership Agreement describes the services and amenities provided through GloFusion Direct Primary care Plan and explains how you may purchase membership. It summarizes the terms and conditions of your membership. By electing to purchase membership in our Primary care Plan, you are deciding to make your health and well-being a top priority.
2) SCOPE OF SERVICES COVERED BY THIS AGREEMENT: The Plan agrees to provide the following services and amenities to the subscribed Members at GloFusion Clinic Griffin in Georgia:
• Unlimited access to your provider
• Unlimited office visits
• Acute/Sick visits
• Well-child checks/Sports Physicals
• Annual physicals
• Chronic disease management
• Medication Management and Prescribing
• Telehealth visits
• Nutrition Counseling
• Discounted Laboratory Rates
• Assistance collaborating with specialist
• An experienced, dedicated staff committed to a high level of care and service
• No appointment necessary for well care or urgent care visits
• Yearly Annual Physical Exam with the following Labs include: Fasting Lipid Profile checking cholesterol level • Urinalysis testing for blood,bilirubin,urobilinogen, protein, nitrite, glucose,PH,specific gravity, ketones, leukocytes • TSH testing thyroid function ketones,
• PSA for men screening prostate cancer.
• CBC/Complete Blood Count testing white blood cell
• Comprehensive Metabolic Panel testing blood count, hemoglobin/hematocrit and platelet count electrolytes, liver, and kidney functions
• Fecal Occult Blood testing which screens for colon cancer
• Annual flu shot: 50% discount
• EKG: 50% Discount
• X-rays: outsourced at a self pay discounted rate
• Ultrasound: Outsourced

MEMBERSHIP PAYMENT
For the above services and amenities, Members will pay an annual membership fee of 59 dollars each year that the Member is in the Plan. After the Patient Enrollment Form and Payment Authorization Form are completed and reviewed, an initial payment must be made in cash or on a debit or credit card. Subsequent payments must be made through debit card, Visa, Master Card, or Discover and automatically charged monthly. The annual fee is $468 per year can be paid monthly at $39, quarterly at $117, or yearly at $468. Family of 4 Includes 2 adults regardless of age and 2 children < 20 $138. Single Parent Family or 3 Includes 1 adult regardless of age and 2 children < 20, $99. Additional children 20% discount.
If your membership begins on December 5th, you will make a payment that day of $39 for the first month's membership fee plus any other fees due under this agreement if being seen the same day. Then on January 5th, your next payment will automatically be deducted according to your preferred form of payment. The membership period is one year. A membership renewal reminder will be sent out in the last quarter of the membership year. For automatic debits and credit card payments, membership will automatically be renewed during the last month of the membership year for the next year. Termination of the GloFusion Direct Primary care Plan must be made by the patient in writing, received 60 days in advance of the annual contract renewal date. Members may change their preferred form of payment by giving a 60 days’ notice.Membership will go into effect upon completion and review of the Membership Enrollment/Payment Authorization Form and completed first payment. If any subsequent payment is not completed successfully, membership will be immediately suspended until the payment transaction is successfully completed. The member (or parent/legal guardian if member is a minor child) is solely responsible for correcting the cause of the transaction failure. Membership will be reinstated on the date the payment transaction is successfully completed and membership will not be retro-active to the date of suspension. If the payment processing issues is not resolved within one week of notification, membership will be terminated as of the date of membership suspension.
HOW TO BECOME A MEMBER
You may become a member in the Direct Primary care Plan by written notification from our office following review and receipt of the completed Membership Enrollment/ Payment Authorization Form, and an initial payment for the first month in cash, debit card, or Visa, Mastercard, or Discover. There will be a $25 late fee for declined credit cards or bank debits. If further action is required to collect fees, the collection and legal fees will be the responsibility of the Member.
BENEFITS OF MEMBERSHIP
You will have access to GloFusion Direct primary care and urgent health care services within its normal business 5 days a week. You will be taking a proactive stance to promote your own health and well-being. Thereby saving you money and time by reducing Emergency room care.
