1. Requests are subject to availability of the Cancer Care Services Outreach Team.

2. To fulfill a request, the Cancer Care Services Outreach Team will need at least at least two weeks notice. During Breast Cancer Awareness month, will we need at least 4 weeks notice.

3. A member of the Cancer Care Services Outreach Team will reach out within 2 business days to confirm availability of request.
