In order for our child advocacy center to be in good standing with the National Children’s Alliance and Children’s Advocacy Centers of North Carolina, our forensic interviewers are required to participate in regular education and peer-review. As part of the peer-review process and in an effort to ensure competent services for our clients, a team of experienced forensic interviewers, who are each bound by a strict confidentiality agreement, will review select interviews conducted by our forensic interviewers and provide constructive feedback. Occasionally, those interviews will also be used for educational purposes.
By signing below, I hereby authorize the SAFEchild Advocacy Center to use the forensic interview portion of my child’s evaluation for peer review and/or training and education purposes, provided that my child’s personally identifiable information (i.e. name and date of birth) is not shared and the interview is not shared with anyone other than those directly involved in the peer-review and/or education process.
I understand that this consent will be valid from the date of my signature below and will expire once the child named above reaches the age of 18.
I further understand that I may revoke this consent at any time by notifying SAFEchild Advocacy Center in writing, except to the extent that information has already been disclosed based on this consent or if continued disclosure is required by law.
