Thank you for your interest in scheduling a Moderna COVID-19 Vaccine at Sharpsburg Pharmacy! Please DO NOT schedule an appointment if you want a different vaccine because other vaccines appointments cannot be scheduled through this form. You must be 18 years of age for us to administer the Moderna vaccine. This is a two-dose vaccine so you will need to return to the pharmacy exactly 28 days after your appointment so that you can receive your second dose. If you cannot return in 28 days for the second dose, DO NOT schedule an appointment until you would be able to do so 28 days later. Because of the dose allocations and storage requirements we simply cannot accomodate requests to reschedule your second dose because of vacations, other appointments, etc.

We recommend that you make a NOTE of your first dose APPOINTMENT Date and Time and DOWNLOAD/PRINT the PDF form for your records once you hit submit. You may do this on the "Thank You" page that appears after your form is submitted successfully. Please bring this and any insurance cards with you for your appointment. An email confirmation of your appointment will be sent but many email providers send Jotform emails to their spam or junk mail folders. Please check these folders before calling the store to confirm.

The CDC's expert advisory committee has recommended a third dose of mRNA COVID-19 vaccines (specifically those by Pfizer and Moderna) for certain immunocompromised people. The third dose for those immunocompromised people listed below should be at least 28 days after finishing the initial two-dose series.
The recommendation does not currently include additional doses for patients who received the Johnson & Johnson (J&J) vaccine or 3rd doses for non- immunocompromised people.
Those who currently qualify for the third dose include people 12 years and older (Pfizer/BioNTech vaccine) / 18 years and older (Moderna vaccine) with medical conditions or who are receiving treatments that are associated with moderate to severe immune suppression, such as:
Active treatment for solid tumor and hematologic malignancies
Receipt of solid-organ transplant and taking immunosuppressive therapy
Receipt of CAR-T-cell or hematopoietic stem cell transplant (within 2 years of transplantation or taking immunosuppression therapy)
Moderate or severe primary immunodeficiency (e.g., DiGeorge, Wiskott-Aldrich syndromes)
Advanced or untreated HIV infection
Active long term treatment with high-dose corticosteroids (i.e., ≥20mg prednisone or equivalent per day continuously for greater than 14 days), alkylating agents, antimetabolites, transplant-related immunosuppressive drugs, cancer chemotherapeutic agents classified as severely immunosuppressive, TNF blockers, and other biologic agents that are immunosuppressive or immunomodulatory)
The additional mRNA COVID-19 vaccine dose should be the same vaccine product as the initial 2-dose mRNA COVID-19 primary vaccine series (Pfizer-BioNTech or Moderna). A person should not receive more than three mRNA COVID-19 vaccine doses.
If you're considered to be moderately or severely immunocompromised and you've been vaccinated with the two-dose series of the Moderna vaccine, you may register below to get a third dose of the Moderna mRNA vaccine.
By signing below and registering for a third dose you are attesting that your medical history aligns with the conditions listed above and falls within the CDC description of being moderately to severely immunocompromised. You will be asked to present your vaccine card showing the first 2 doses of the Moderna vaccine and you may be asked to confirm your medical history aligns with the conditions outlined above if the pharmacy does not have record of immunosuppressive therapy.
Fully vaccinated people with healthy immune systems do not need another dose of COVID vaccine at this time. If you aren't sure whether you qualify for a third dose, please talk to your healthcare provider or call the pharmacy.
In addition to a third dose of mRNA COVID-19 vaccines, immunocompromised people should continue to take precautions such as wearing masks, avoiding crowds, and keeping their physical distance from people they don't live with. Likewise, everyone around immunocompromised people should get vaccinated against COVID-19 to protect their loved ones.

Please note that we will call wait list patients on short notice and you must be able to come to the pharmacy quickly to help us prevent wasted vaccine.
If you need to cancel an appointment please call us so that we can open the spot up for someone else. No shows for appointments can mean that we have to waste a dose that day. Please help us reach as many patients as possible by keeping your appointment time or giving us plenty of notice if you can't.
