🦽 Vacation Planning Disability Checklist

In order to provide you with a great cruise experience we ask our guests with special needs to complete this form at the time of booking or no later than 5 days prior to sailing except for sign language requests which must be submitted 90 days prior to sailing. Although you are not required to provide information about the extent of your disability, the more information we have the better we can accommodate your specific need.

Although we don't require you to answer this section, it is helpful for us to understand your accommodation needs, especially with children and some adults.  This will help us when working with our vendors to determine what we can do to enhance your vacation.

If you intend to bring a Service Animal on your vacation, we wish to advise you that it is your responsibility to obtain and provide the following documentation no later than 7 days prior to sailing for clearance.

All customs and other governmental approvals to disembark your service dog in the various ports of call, including a USDA certificate from the veterinarian.

All necessary vaccinations for the return of your service dog into the US and UK.
Please be aware of the following:

• You must bring the service animal’s food and medications (if any), as well as life jacket.
• The animal must always be attended to and cannot be left alone in the stateroom or anywhere else on the ship.
• A relief sand box will be provided.

In accordance with the Americans with Disabilities Act, Norwegian Cruise Line does not accept emotional support dogs.
