  • Important Information and Submission Directions

  • The KBG Scholarship must be completed and submitted by May 8, 2021.  You are able to save this application form and revisit it as needed. Please download the below files and send them to the appropriate appraisers.  Ask that the appraisers send you back the completed form so you can upload them to this scholarship submission form.

    Additionally, ask your Guidance Counselor to email a copy of your transcripts to Matt.Coyle@lung.org to be included with your scholarship application.

    If you are awarded a KBG Scholarship, you will have until May 31, 2021 to submit needed information to process your check to be sent to your college/university. If you do not sent your high education's information, KBG will be unable to process the check.

    Personal Statement

  • In a separate document that will be uploaded to this form, provide a type-written, double-spaced, personal statement with no less than 12 font type, in 500 words or more.  In the top left corner provide the following information:

    Name

    County 

    The intent of the personal statement is to allow the scholarship committee with the opportunity to learn more about your leadership and volunteer service in tobacco control.

    This can be a tough assignment as you stare at a blank page and decide on how to convey your contributions to the fight against the devastation tobacco use causes.  As you decide what to include and not include consider these:

    1. Give a brief statement about yourself, which gives insight into you as a person.  Consider sharing something interesting or unusual that happened to attract you to the movement against tobacco.  Share any important family history.
    2. Tell us what is important to you.  Why are you involved in the fight against tobacco?  Who and what does your work affect?
    3. Finally, tell us why this scholarship is important to you.

  • Transcripts must be received no later than May 1, 2022.  Your transcripts can sent electornically via email by your school counselor.  Please have your counselor email transcripts to Matt Coyle at Matt.Coyle@Lung.org. Transcripts and subsequent applications that are received after May 20, 2022 will not be considered for the KBG Scholarship.

    By signing below, you also acknowledge that if you have been chosen as a recipient of the KBG Scholarship, you will turn in the required information for your college/university (information will be requested upon notification of scholarship awarded) by June 1, 2022, or your scholarship may be rescinded.
