Important Notice for Medicare Patient - Advanced Beneficiary Notice

The Cornea and Laser Eye Institute strives to give the highest quality of care to our patients.  Medicare requires us to inform you that there are some services that are not covered benefits by Medicare.  These services include: 

Refraction:  Test used to measure your eyes for glasses for a prescription.  If we give you a presription for glasses, there is a charge of $85 payable the day of your appointment.  

Medicare Advantage Plan: If you have a Medicare Advantage Plan such as Horizon Medicare, United Healthcare Medicare, etc., we only participate with these plans if you have a PPO plan.  If your card states HMO or EPO, we do not participate in your plan and you would be responsible for all services, also including co-pays and deductibles.  

Medicare without a secondary (supplement) insurance: If Medicare is your only insurance, you will be responsible for the 20% coinsurance for all services, also including co-pays and deductibles.  

Medicaid: CLEI is not a participating provider of Medicaid.  If you have Medicaid as a secondary insurance to Medicare, you will be responsible for 20% coinsurance that Medicare does not cover. 

To maintain the highest level of care to our patients, we request these non-covered services be paid-in-full at the time of service.
