CREDIT REPORT AUTHORIZATION, PRIVACY DISCLOSURE AND SUBMISSION OF CREDIT APPLICATION FORM

Car Credit Solutions Inc and SAFE FINANCE LLC (hereafter “Car Credit”). 

I certify that the information in this application is complete and correct and that is my intent to submit a credit application to (Car Credit) for the purpose of obtaining vehicle financing. 

I understand that (Car Credit) will retain this application whether or not it is approved, and that it is my responsibility to notify (Car Credit) of any change of name, address or employment. 

My signature below also authorizes the release to credit reporting agencies of financial or other information that I have supplied to (Car Credit) in connection with such evaluation. Authorization is further granted to the credit reporting agency to use a copy of this form to obtain any information the credit reporting agency deems necessary to complete my credit report. 

I authorize (Car Credit) to investigate and obtain information regarding my employment history, current and previous. I also authorize (Car Credit) to obtain information regarding any past or present residential rental accounts. 

This authorization to access this information and to contact you regarding your account shall remain in effect as long as any balance remains on any account owned by the undersigned unless prohibited by state law in which case this authorization will continue in effect for the maximum period allowed by law.
