Declaration of Major, Concentration, & Minor
Use this form to declare or change major, concentration, and/or minor.

You will be required to meet all program requirements, including general education curriculum, in effect at the time of transfer as listed in the current academic catalog. Changing programs or declaring a minor or concentration may result in additional units required beyond the minimum to graduate in your current major. Credits earned in your current major will be reviewed for applicability to the new program when the request is processed.

This request is subject to approval by your department chair and/or advisor. You should consult with your advisor and the chair of the new program, if applicable, prior to submitting your request.

Graduate students: You have been admitted to a specific department and degree level, and program transfers between departments and/or degree levels are not allowed. To change your program to one offered by another department or to change your degree level, you need to apply to and be accepted to the program of interest. The Admissions department will assist with the application and your enrollment will be updated upon receipt of A] an approved application for admission to the new program and B] a withdrawal form from your current program. Enrollment in the new program will take effect the first day of the next applicable term.
