This practice strives to provide the highest quality of care while attempting to accommodate each patient's schedule. Consistent attendance and adherence to the planned treatment regimens are paramount to your care and recovery. Your appointment time has been reserved for you. I understand that an emergency may occur and life can sometimes get in the way of plans. However, missed appointments are detrimental to all parties involved. Therefore, I have developed a "client attendance policy" explained
A "no show" is defined as an absence of the appointment without notice. A "late cancellation" is defined as providing less than 24 hours' notice of the client's inability to keep the appointment. Please cancel the appointment at least 24 hours prior if you are unable to keep your scheduled appointment for any
Three "no shows" or "late cancellations" during an episode of care will be considered a pattern of missing appointments. I will review the client's treatment plan and reserve the right to decide if it is appropriate to continue providing service to this client. If a client is discharged from the practice due to a pattern of missed appointments, they will be responsible for obtaining a new provider (Due to the nature of the termination, I will not refer them to a colleague
Any new client who fails to schedule and attend their follow-up visit after the initial intake appointment will generally be discharged for lack of follow-up. A rescheduling opportunity will not be offered.
