Is the patient 65 years of age or older, 18-64 years of age at high risk of severe COVID19, or an individual 18-64 years of age whose frequent institutional or occupational exposure to SAR-CoV-2 puts them at risk of serious complications of COVID19 including COVID19?

Did the patient previously receive the Pfizer vaccine?

Patient is eligible for a Pfizer booster dose ≥ 6 months after completion of Pfizer series

The following individuals should receive a Pfizer booster dose:

≥ 65 years old

Long-term Care Facility residents

50-64 years of age with underlying medical conditions

The following individuals may receive a Pfizer booster dose:

18-49 years of age with underlying medical conditions

18-64 years of age who are at increased risk for COVID19 exposure and transmission due to occupational or institutional setting (healthcare workers, teachers, and other essential workers)

Examples of immunocompromised patients include those who have:

Been receiving active cancer treatment for tumors or cancers of the blood

Received an organ transplant and are taking medicine to supress the immune system

Received a stem cell transplant within the last 2 years or taking medications to suppress the immune system

Moderate or severe primary immunodeficiency (such as DiGeorge syndrome, Wiskott-Aldrich syndrome)

Advanced or untreated HIV infection

Active treatment with high-dose corticosteroids (≥20mg Prednisone or equivalent) or other drugs that may supress your immune response

Patient is eligible for third dose ≥ 28 days after completion of Pfizer or Moderna series (Third dose must be the same vaccine they previously received).
