By sending this electronically, I acknowledge that I have completely read this questionnaire and comprehend it fully. I understand that applying does not ensure approval and that untruthful answers or failure to comply with the requirements of this application can result in the forfeiture of any Foster2Forever animal fostered by me. I certify that the above information is correct, and I understand that the information will be verified. I understand that by submitting this form electronically, I agree to release and covenant to hold harmless Foster2Forever and it's members from any claims, damages, costs, or actions incurred because of the care or actions of the foster animal. If the foster individual provides medical care without prior authorization from Foster2Forever, they will incur the charges. I accept full responsibility for the animal's actions at all times, and release Foster2Forever from any liabilities or damages that may be incurred because of fostering such animals. I agree to have Foster2Forever complete reference call checks and conduct a home visit inspection to be able to approve my foster application. I agree that if Foster2Forever views my house or anyone in my residence as an unfit foster parent I will return the animal safely to any the board members of the rescue upon request. If I'm unable to foster the animal anymore that I will return the animal to Foster2Forever and try to give Foster2Forever a 2 week period to try and find another suitable foster for the animal.Foster2Forever is not opposed to fosters adopting fosterlings, although by allowing them to find a new forever home, fosters can help yet another animal in need. Our foster families say all the time that despite it being hard to say goodbye to a fosterling, it's all worth it in the end when you find that perfect adoptive family and you know the animal lives happily ever after! If fosters find that they’re absolutely smitten with their fosterling and can’t bear to live without him/her, we request that the decision to adopt be made within two weeks of the animal arriving into the rescue and we be notified before we put the animal up for adoption publicly. Once he/she is up for adoption, if there is an interested party, they will have the first right to adopting, and we will no longer guarantee adoption to the fostering family. All families wishing to adopt must pay the required adoption fee.
