Inspired by the love and grace of Jesus Christ, Grace Medical Home is a medical practice for the uninsured. Our staff and volunteers partner with you to address your unique health care needs including primary care, specialty care, coordination of care, community resources, medications, spiritual care, counseling and other health and wellness needs.
We want to support and encourage your health journey any way we can. Please let us know of any potential obstacles that may hinder your care such as language, work or transportation issues.
Everything at Grace is provided by the generosity of community donors and volunteers and we strive to be good stewards of these valuable resources. As a partner in managing our health care resources, we expect our patients to show the same respect and stewardship of our community’s investment by following our patient expectations. Failure to do so may result in being discharged as a patient at Grace Medical Home.
1. EXHIBIT APPROPRIATE BEHAVIOR
We hope and expect all people will be treated with dignity and respect while at Grace.
· Rude behavior will not be tolerated. If your conduct is such that a staff member/volunteer complains or is upset, you will be notified in a warning letter.
· Extremely rude behavior or abusive, foul language may result in immediate discharge.
2. SHOW UP FOR YOUR SCHEDULED APPOINTMENT
We expect all patients to honor your appointment times, just as Grace respects your time and work schedule.
· Please arrive at least 15 minutes prior to your scheduled appointment; if you are late for your scheduled appointment, your provider may not be able to see you and you may be asked to reschedule to a later date.
· Your first visit with a provider is vital to establish you as a patient at Grace. If you miss your first appointment with the physician without notification, you will be immediately discharged as a patient.
· It is the responsibility of the patient to remember and/or cancel the appointment. If you need to cancel your appointment, please call at least 48 hours in advance. You may call anytime and leave a message if you are unable to speak with someone. Failure to call to cancel your appointment will result in the appointment being treated as a no show.
o As a courtesy, you will receive a reminder text or automated voice mail usually 2 and 3 days prior to your scheduled appointment. When you receive our reminder, you will have the option to confirm or cancel your appointment immediately.
· We heavily rely on the generosity of our community volunteer providers who donate their time and services to help care for our patients. Therefore, not showing up for an appointment with a primary care provider or specialist may limit your access to specialty care in the future and/or result in discharge as a Grace patient.
· From time to time, providers may need to cancel or reschedule an appointment due to unforeseen circumstances. Should this occur, we will notify you as soon as possible.
3. COMPLY WITH TREATMENT PLAN AND RECOMMENDATIONS
The staff and volunteers at Grace Medical Home strive to do what is best and right for our patients by providing the highest quality care. Please let us know if you have any concerns about the recommended treatment plan so that we can work together to determine a mutually-agreed upon course of action.
· As an integrated practice it is expected that you comply with the recommended treatment by any of our providers including medical, mental health and dental care. Failure to do so will result in receiving a warning letter and may ultimately result in being discharged from Grace Medical Home.
· To ensure consistency in care, we request that you do not use an additional primary care provider or clinic simultaneously while being a Grace patient.
· Part of compliance is keeping contact information updated. It is your responsibility to inform us if your contact information has changed.
4. RE-REGISTER AS A PATIENT ANNUALLY
· It is the patient’s responsibility to re-register each year as mandated by the Florida Department of Health. If a patient fails to re-register within 12 months of the original registration date, they will be discharged and any scheduled appointments will be cancelled.
o As a courtesy, you will receive a reminder text notice a minimum of two weeks prior to your expiration date. If you have not scheduled an appointment to re-register, you will also receive an additional text message notifying you of your upcoming discharge date. Once a patient’s eligibility has expired, a discharge text is sent.
o You will be expected to provide proof of income. This can be, but not limited to, pay stubs, letter of employment or bank statements.
· If you are discharged as a Grace patient and wish to re-enroll, you must follow the same procedures as other potential patients.
5. NOTIFY GRACE IMMEDIATELY OF ANY CHANGES IN:
· HEALTHCARE COVERAGE (private insurance, Medicaid, Medicare, VA benefits or disability) According to the Florida Department of Health guidelines, we cannot treat patients with insurance. If you obtain insurance privately or through a government program (Medicaid or Medicare) or if you obtain healthcare coverage through benefits such as disability or the VA system, you can no longer be a patient at Grace Medical Home.
· IN THE EVENT OF A PREGNANCY
We are unable to provide pregnancy care at this time. If you become pregnant, you will need to obtain care with another medical facility. Please notify us as soon as possible so that we can assist in a smooth transition to your new medical provider. You may re-apply at Grace Medical Home after the end of your pregnancy or if you lose insurance coverage. To facilitate transferring your care, Grace Medical home will remain available to treat you for acute medical care for thirty (30) days following the date of the initial notification of pregnancy or insurance coverage (restrictions may apply).
· INCOME
While we would like to help, we are not able to assist those who earn over 200% of the federal poverty guidelines. If this is your case, please notify us and we will begin the discharge process to end your care with Grace Medical Home.
6. FIND ADDITIONAL HEALTH CARE IF DISCHARGED
· If discharged, you should place yourself under the care of another physician promptly. To facilitate transferring your care to another physician, we will remain available to treat you for emergency medical care for thirty (30) days following the date of the discharge letter (restrictions may apply).
Please make the transfer to a new physician as quickly as possible within that time period.
I ACKNOWLEGE THAT I HAVE READ, UNDERSTAND AND AGREE TO ABIDE BY THE POLICIES AS SET FORTH BY GRACE MEDICAL HOME IN ACCORDANCE WITH THE FLORIDA DEPARTMENT OF HEALTH. FAILURE TO FOLLOW THE ABOVE GUIDELINES MAY RESULT IN RECEIVING A WARNING OR DISCHARGE LETTER.
