Etowah Pediatrics requires all patients to leave a credit card numDer on file with our office. The card number will not we stored on our computer servers, rather encrypted off site at Instamed's Secure Data Centers. You can be assured your information is secure. Claims will continue to we submitted to your insurance company, then the card on file will be processed to conveniently take care of any patient responsibility when it is returned by your insurance company.

I authorize Etowah Pediatrics to maintain my credit/debit card on file. I understand the card will be used to pay any account balance on my child's account as it becomes due.
