    Please note, we are only performing COVID testing for travel purposes, because our COVID tests are only validated for asymptomatic populations without recent COVID contact. To ensure that we are using the tests properly, please bring your passport if travelling (other IDs OK if for work), as well as your travel itineraries (or work document requesting asymptomatic testing).

    Pay in-person at the time of the appointment. No booking, cancellation, or change fees. If you wish to make last-minute changes to your test or add-on options, please make it very clear at the time of your appontment.


  • Screening Questions


    Please call 911 if you are experiencing severe difficulty in breathing, severe chest pain, change in the level of consciousness, or any other significant discomfort. 


    Our rapid COVID-19 screening for travelers is approved for ONLY asymptomatic travelers by Health Canada. You must go to a public testing site if you have the following symptoms:


    Chills or significant tiredness/fatigue/lack of energy


    Fever of 37.8°C (100°F) or higher


    Sore throat, cough, barking cough, or recent worsening of wheezing


    Shortness of breath or difficulty/pain swallowing


    Decrease or loss of taste or smell 


    Recent onset runny or stuffy/congested nose 


    Pink eye


    Digestive issues like nausea/vomiting, diarrhea, stomach pain


    Recent onset headache or muscle aches


    Recent contact with patient confirmed or suspected of having COVID-19


    Currently under mandatory or recommended quarantine due to positive result, recent symptoms, or border entry status.

  • Patient Email and Electronic Communication Consent Form

    Transmitting patient information via email and other eletronic means poses risks, and you should not consent to communicating with us regarding your healthcare matters before fully understanding and accepting these risks. Such risks include, but are not limited to:

    1. The security of electronic communications cannot be guaranteed, and their use may increase the risk of sensitive information being leaked.
    2. Employers and online services may have legal rights to inspect and retain electronic information that passes through their system.
    3. Electronic communications may be easier to falsify than traditional means. It may be difficult to verify the true identity of the sender, or to ensure that only the intended recipient have access to the content. Choosing not to use encryption software further increases that risk.
    4. Electronic information can be intercepted, circulated, stored or changed without the knowledge or consent of the clinic or patient. It can be easy to misaddress electronic communications, resulting in unintended leaks to unknown recipients. 
    5. Emails may introduce viruses and potentially damage or disrupt the computer.

    Rapid Clinic Canada will use all reasonable means to protect the security and confidentiality of its electronic communications. However, because of the risks outlined above, you agree to the following conditions:

    1. You must use your own electronic devices for electronic communications with Rapid Clinic Canada, and use passwords whenever possible to limit access.
    2. You must inform us of any changes in your contact information, and respond to electronic communications in a timely fashion.
    3. For serious, life-threatening, or rapidly evolving conditions and situations, you must not rely on electronic communications. Electronic communication is not an appropriate substitute for clinical assessments. If you have not received a response within a reasonable time period, it is your responsibility to follow up on this via other means.
    4. Electronic medical information may be stored as part of your medical record and shared with healthcare workers who play a role in your "circle of care" and staff with authorized access.
    5. In cases of technical breaches, Rapid Clinic Canada or its staff shall not be held responsible.

    Patient acknowledgment and agreement

    By clicking the consent button below, I acknowledge that I have read and fully understand the content of this form. I acknowledge the right of the clinic to, upon the provision of written notice, withdraw the option of communicating through electronic means. I agree that the clinic team and physicians, staff, directors, officers and other agents shall not be responsible for any personal injury including death, and/or privacy breach (outside the reasonable control of the clinic) or other damages as a result of my choice to communicate electronically with the clinic or its staff and I release the clinic and their physicians, directors, officers and other agents from any liability relating to electronic communications.
