The office is contracted with multiple insurance companies. We will, as a courtesy, call and verify benefits, however, it is the responsibility of You, the Patient, to make sure that the provider you are seeing is contracted with your insurance company, otherwise you will be responsible for charges stipulated by your insurance company. We will not become involved in disputes between you and your insurance company regarding deductible amounts, copays, covered charges, secondary insurance, pre existing conditions, etc.
Please keep in mind that benefits quoted by your insurance company reflect benefits in place at the time of eligibility and do not guarantee payment, therefore you will be held responsible for charges not covered by your insurance company or incurred before/after eligibility.
Benhaven Counseling 2, LLC does not accept the following insurances: Medicaid. If you switch to a Medicaid coverage your services at Benhaven will not be covered and you may have to become a self-pay client. We do accept CareSource, UnitedHealthcare, Buckeye and Paramount (which are Medicaid HMO's and not straight Medicaid Co-Payments, Co-Insurance, Deductibles Co-Payment is a payment defined by your insurance company as a set amount of money you must pay each time medical services are rendered.
