    Any pickup persons must share the family passcode in order to checkout any student. Furthermore, the family passcode will be used to verify your identity before information is shared via phone. Please input your current code if you do not wish to update your family passcode.
  • Please notify BOLPA if an Emergency Release Person will pick up your child on a given day. For the safety of your child, we will require all authorized release persons to provide Government-issued photo identification at the time of pick-up. All persons below must be 18 or older, unless he/she is the parent of the child. All pickup persons must know the family passcode.

    The person (s) designated as emergency pickup people will be contacted and are authorized to pick up my child if there is a medical or other emergency and I cannot be reached. Parent/Guardian must complete any state-specific emergency release form required by individual state childcare licensing regulations.
    BOLPA staff will release your child only to you or those persons you have listed above. Emergencies may prevent you from picking up your child; therefore, include those individuals whom you would authorize in such events. If you want a person who is not identified above to pick up your child, you must notify school staff in advance, in writing. Your child will not be released without prior authorization. In the event you call a pick-up authorization into BOLPA because you are unable to submit your authorization in writing, we will use your personal information to verify your identity.

     

    Enrollment Fees

    I understand that an annual, non-refundable Enrollment Fee shall be paid in advance to enroll my child. I understand that I may guarantee my child’s enrollment by paying this fee in advance each year. In instances of agency reimbursement, the Registration Fee is to be paid according to the applicable contract. Please note that all fees are non-refundable. 

    Please note that re-enrollment may be denied, and a refund may be issued based on your scholar's behavior, academic performance, and family commitment.

     

    Tuition Modifications and Payments

    I understand that rates are subject to change with reasonable notice as conditions require. BOLPA follows state-specific required time frames on tuition and modifications notices. I understand that tuition is due and payable, on the first day of attendance each week. Appropriate alternate Tuition Fees must be paid during preschool breaks.

    I understand that because tuition is a flat rate for elementary and Jr. Kindergarten students that may be divided monthly as a courtesy, I am still responsible for the entire yearly tuition amount, regardless of un-enrollment, expulsion, or any COVID-19 related matters. Furthermore, a fee may be required for preschool students if BOLPA is closed due to inclement weather, natural disasters, or COVID-19.

     

    Late or Unpaid Tuition

    If payment in full is not received when due, I agree to pay a late payment fee of $25 per week that tuition is not received. All late fees are subject to change with reasonable notice. I understand that if my account is delinquent for more than one week, I may be asked to withdraw my child until my account is made current. BOLPA cannot guarantee a child’s spot will be held when a child is withdrawn due to non-payment of tuition. Any unpaid tuition fees may be sent to a third-party collection agency.

     

    Agency Reimbursement

    I understand that I am solely responsible for any tuition payment and late fees in excess of any agency or third-party reimbursement in accordance with the applicable contract. I also understand that I am solely responsible for promptly communicating any changes in my status that would affect my agency reimbursement, and that I am solely responsible for payment of any tuition in excess of any agency or third-party reimbursement resulting from my failure to promptly communicate status changes.

     

    Late Pickup

    BOLPA is open from 7:00 AM to 5:00 PM, Monday through Friday all year, except for holidays. I understand that if I fail to pick up my child by the scheduled closing time, I will be charged a late fee of $2.50 per minute until the child is picked up. Please note that this late fee may change.

     

    Returned Checks

    I understand that a processing fee of $25 per check will be charged to my account for all checks which are returned for any reason, and this fee is in addition to any charges that my bank or financial institution may charge me. I understand that any non-sufficient funds checks will be automatically resubmitted electronically up to three times. I further understand that once a check has been processed electronically, the check is no longer negotiable and will not be returned. If more than two checks are returned within a six-month period, I will be required to pay by an alternate method of payment for the next six-month period. If my school uses Telecheck, I am authorizing the payee, or its agent, upon receipt of my check, to convert the check to an electronic payment item or draft and to submit it for payment as an ACH debit entry or draft to my account, in accordance with the same terms and conditions as my check. In the event that my check is returned for non-payment, Telecheck will make up to two additional electronic collection attempts and, if needed, by paper draft thereafter. The maximum fee allowed by state law will be charged for all returned checks. I am responsible for the principal amount plus all returned check fees.

     

    Illness

    I understand that I will be notified should my child become ill during the day, and that I will pick up my child promptly, or make arrangements for an authorized emergency contact person to pick up upon such notification within one hour. If my child is exposed to or contracts a contagious disease or virus, I agree to notify BOLPA and I understand that my child will be re-admitted according to the Re-admission Criteria in the Family Handbook.

     

    Photographs, Videos, and Audio Tapes

    I understand and agree that, in consideration for being allowed to photograph, videotape or audio record my child on company property, I shall only use such recording for lawful and private home use, and will not publish, publicly display or sell such recordings. I also understand that I must have written permission before capturing any image of the other children in BOLPA or staff.

     

    Interviewing Children & Inspecting Records

    I understand that the state child care regulatory enforcement and administration agency and the local depart¬ment of social services or child protective services has the authority to interview children or staff, to inspect and audit child or facility records, to interview children privately, to observe the physical condition of the children in BOLPA, to make provisions for the independent medical examination by a licensed physician of any child, and to contact and instruct any other appropriate authority to do the same, without prior notice or consent by myself or by BOLPA.

     

    Withdrawal from Preschool

    I understand that I must provide a two (2) week written notice of withdrawal from the program. If this notification is not provided, I agree to pay all tuition and fees for two (2) weeks, whether or not my child attends. I understand that when my child is withdrawn, s/he will only be eligible for re-admission based upon space availability and all other enrollment criteria. If my child is selected for re-enrollment, I will be required to complete an entire new Enrollment Agreement at the current rate and pay a new non-refundable Registration Fee at the current rate. If there is an outstanding balance (including tuition or fees) when my child was withdrawn, I will be required to bring my account current prior to completing a re-enrollment application. I understand all fees (Tuition, Registration or Activity) are non-refundable.

     

    Holidays

    I understand that BOLPA is closed on the following holidays: New Year’s Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, Christmas Day, as well as either Martin Luther King, Jr. Day or President’s Day for in-service training. I also understand BOLPA is closed the week of Christmas in observance of the holiday. I agree that I will not receive a refund, credit or any other allowance for holidays. If a holiday falls on a weekend, it will be observed on either the preceding Friday or the following Monday.

     

    Preschool Absences

    I agree to inform BOLPA immediately if my child will be absent on any day. I understand that no allowances, credits, refunds, or make up days shall be made for occasional absences (i.e. sickness). A reservation fee of 50% off my regular week’s tuition will be due for each absence of one full preschool week (Monday through Friday) with advance notice to the Director, if possible. I agree to pay the reservation fee of $60 per week to guarantee my child’s space when my child is not in attendance for an entire preschool week (Monday through Friday). My regularly contracted tuition is due for all weeks when my child attends any part of the week. There is no credit given for single days. I also understand that if I withdraw my child during a vacation, I will be required to pay a new non-refundable registration fee upon return.

     

    Inclement Weather & Disasters

    I understand that it is the company’s intention to be open and provide childcare service every weekday of the year, excluding holidays, but that inclement weather, natural/national disaster or major building issue may disrupt service from time to time. I will contact BOLPA to ensure that it is open during inclement weather/natural disaster. I agree that in the event that BOLPA is closed for an extended period of time, I will continue to be responsible for my tuition payments for up to three business days.

     

    Parent Handbook

    I understand that I am responsible for obtaining BOLPA’S parent handbook online at www.bol.academy, with the option of printing or viewing online at my leisure.

     

    Registration Documents 

    I acknowledge that there are additional documents that must be downloaded, completed, and submitted to BOLPA before my child may begin school.

     

    Lost or Damaged Property 

    BOLPA will employ the Word of God in its teachings and principles provide a safe environment and use reasonable, sound judgment in overseeing my scholar while entrusted to its care. Nonetheless, I understand that accidents occur. BOLPA is not responsible for lost or damaged items that children may bring to the property. I, therefore, hold harmless BOLPA, its agents and employees from all claims, damages, or other liabilities for lost or damaged property that my scholar may bring to school and for injuries to my child which are not the result of gross negligence by BOLPA, its agents and employees. By the same token, BOLPA realizes that scholars will accidentally damage property. Notwithstanding, if my scholar willfully and/or wantonly defaces property, I agree to pay BOLPA to restore such property.

     

    COVID-19

    I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing.

    I agree to inform BOLPA if my child or someone in my/my child’s household should become ill or display any symptoms
    related to COVID-19, including; cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills,
    muscle pain, headache, sore throat, or new loss of taste or smell. Furthermore, I agree to inform BOLPA if my child has been exposed to any contagious disease or virus in any way.

    I further acknowledge that Breath of Life Preparatory Academy has put in place preventative measures to reduce the
    spread of the Coronavirus/COVID-19.
    I further acknowledge that Breath of Life Preparatory Academy cannot guarantee that I or my child will not become
    infected with the Coronavirus/Covid-19. I understand that the risk of becoming exposed to and/or infected by the
    Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited
    to, BOLPA’s Staff, and other students and their families.

    I voluntarily seek services provided by Breath of Life Preparatory Academy and acknowledge that I may be increasing my
    child’s risk to exposure of the Coronavirus/COVID-19. I acknowledge that I must comply with all set procedures to reduce
    the spread of COVID-19 while my child attends BOLPA.

    I hereby release and agree to hold Breath of Life Preparatory Academy harmless from, and waive on behalf of myself, my child, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses, and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to
    act of BOLPA, or that may otherwise arise in any way in connection with any services received from BOLPA. I understand
    that this release discharges BOLPA from any liability or claim that I, my child, my heirs, or any personal representatives may
    have against BOLPA with respect to any bodily injury, illness, death, medical treatment, or property damage that may
    arise from, or in connection to, any services received from Breath of Life Preparatory Academy. This liability waiver and
    release extends to BOLPA together with all owners, partners, and employees.

  • Elementary Enrollment Contract & Tuition Obligation

    Breath of Life Preparatory Academy agrees to enroll the eaglet named above for the current academic year and to provide the educational program prescribed at the appropriate grade level.

    In consideration of the acceptance of the Enrollment Contract by BOLPA, I agree to pay the required tuition and all other miscellaneous fees that I incur due to my child’s enrollment at BOLPA. A $25.00 fee will be assessed for processing notes with insufficient funds.

    I understand that an annual, non-refundable Enrollment Fee shall be paid in advance to enroll my child. I understand that I may guarantee my child’s enrollment by paying this fee in advance each year.

    I understand that because yearly tuition is a flat rate that may be divided monthly as a courtesy, I am still responsible for the full tuition amount, regardless of any COVID-19 related matters. Furthermore, a fee may be required for preschool students in the event that BOLPA is closed due to any COVID-19 related matters.

    I understand that my obligation to pay tuition and other fees for the full academic year is unconditional and that upon the day which BOLPA has been in session 15 days or on the 15th day that my eaglet has been enrolled at BOLPA, no portion of fees paid or outstanding will be refunded or canceled in the event of absence, withdrawal, or dismissal from BOLPA. Also, no portion of fees will be returned or canceled if my scholar is not able to attend or participate in activities.

    Payments more than 30 days past due may result in my child’s removal from class until my account has been made current. All accounts must be in good financial standing by the end of April. No balances can remain after the close of the current academic year.

    Conflict Resolution

    BOLPA considers its relationship with families of BOLPA eagles to be one of mutual respect and kinship. Therefore, BOLPA requests that parents exhaust all internal means (BOLPA administration) to settle disputes before seeking external resolution.

     

    Corporal Punishment

    In the interest of correcting misconduct, parents may give BOLPA permission to administer corporal punishment for their child’s disobedient behavior and disrespect for those in authority. Parents are not required to agree to corporal punishment and may be contacted to pick up their child immediately if sent to the office for misconduct. Corporal punishment, is administred under open rebuke, parents may opt for corporal punishment with or without open rebuke.
