Option #1: Receive PB Laptop

Liability for damaging or losing equipment

In acceptance of this device (Laptop), I agree to the terms and conditions stated below:

I understand that I am responsible for the laptop and keeping it in good condition while it is in my possession.
I understand that privacy and security are important and that I will not install any additional programs or software that are not approved for use by PB.
I am the only authorized person to have access to and to use this laptop. Any unauthorized access to this laptop is a violation of PB policy, employment regulation and employment contract.
In the event of loss, theft, or damage, this must be reported to the police within 24-48 hours, a copy of a Police or incident report must be submitted to PB for verification purposes.
I understand that any violation of these terms and conditions is subject to disciplinary action by PB.
When my employment is terminated, I will remove all personal, non-work related data from the computer before returning it to PB.

Option #2: Don’t Receive PB Laptop

I acknowledge that I was offered a PB laptop.
I acknowledge that I voluntarily declined the offer of receiving a PB laptop.
I acknowledge that my own personal computer is sufficient to do the work requested of me, and the use of a PB laptop is not necessary to perform my duties.
