As a convenience to job applicants and our employees and in an effort to reduce waste, as well as to implement measures for completing employment-related documentation for applicants applying and employees working remotely, Francis Parker School (“the School”) provides applicants and employees with forms and agreements in electronic format, and requests applicants and employees sign certain documents electronically through Jotforms, Adobe Sign (henceforth known as Electronic Document Management System EDMS) or similar EDMS. Documents executed electronically will be treated in the same was as a physical handwritten signature on a hardcopy paper form. This Agreement is intended to provide you with the details regarding the School’s Electronic Signature (ES) process and describes your obligations with respect to your ES.
By including your signature below you agree to and acknowledge the following:
Consent to Electronic Delivery:
I agree to receive, obtain, and/or submit any and all School documents and information electronically, including documents and information related to my application and/or employment with School.
Consent to ES:
I agree that my ES will be valid for the duration of my pending job application and employment (if I am hired or if I am a current employee), or earlier if it is revoked or terminated per the terms of this Agreement. I agree and consent that my use of a keypad, mouse or other device to select an item, button, icon, checkbox, to enter text, or to perform a similar act/action, (e.g., by clicking “I agree”) constitutes my ES and signifies my intent to be bound. I understand that my ES signature is legally binding, just as if I manually signed a paper document in ink.
Security Requirements and Authorized Use:
I am solely responsible for protecting my ES from use by anyone but me. I understand that whenever I electronically sign and submit an Electronic Document through an EDMS, I will receive an email at my email address on file with the School informing me that a submission has been made to the School through EDMS from my email. I agree that if I receive an email notification for any activity that I do not believe I performed, I will notify the Manager of Benefits and HRIS in writing at Lcaouette@francisparker.org as soon as possible, but no later than 24 hours, after receipt. I further agree to contact Human Resources if I do not receive an email notification within three business days for any electronically signed submission using my credentials.
I agree to report, within 24 hours of discovery, any evidence of discrepancy between any Electronic Document I have signed and submitted and what the School has received from me by contacting the Manager of Benefits and HRIS in writing at
If I discover or suspect that my ES has been or is in danger of being lost, stolen, disclosed, compromised or subjected to unauthorized use in any way, I will immediately and no later than 24 hours notify the Manager of Benefits and HRIS in writing at Lcaouette@francisparker.org. I will then immediately cease all access and use of my ES.
I understand there may be situations that will require me to immediately cease using my ES. These include but are not limited to the following situations: If I have requested that my ES be revoked; if I am notified that someone has requested that my ES be suspended or revoked; if my job application is no longer pending and I am not offered employment with the School; if I resign or my employment with the School is otherwise terminated; and/or if I am no longer required or permitted to utilize my ES for any other reason. Under these situations and any others as identified by the School, I agree to and will immediately cease using my ES.
Receiving Paper Copies:
I understand that I may request from the School a paper copy of any record made available electronically to me. I may request delivery of paper copies from the School by contacting the Manager of Benefits and HRIS.
I may decide at any time to receive documents related to my employment in paper format from the School by notifying the Manager of Benefits and HRIS in writing by mail or electronic means.
