Students attending a Mountain Sage event must abide by the following guidelines:
- Submit this completed form at least one week prior to the date of the event
- All guests must present proof of the confirmation email from the school; this will serve as the ticket for entry
- Student must enter and leave with their guest
- For dances: all guests must be under the age of 15
- For other school events: all guests must be under the age of 18 or an approved volunteer at MSCS
- All guests agree to comply with the rules and guidelines that are outlined in the Mountain Sage Community School parent handbook while they are attending an event
- All guests will be kind, safe, respectful, and responsible to themselves and others
- Guests agree to listen and comply with appropriate behavior as outlined by staff
- Guests agree to follow all school health related requirements, including any stated COVID-19 precautions if needed (proper use of masks, social distancing, etc)
