Payment Policies:

No Insurance:

Payment is required, in full, on day of service. We offer payment by cash, check, credit card, and debit.

Dental Insurance:

Options:

1) Pay for your services, in full, on day of service. We submit your insurance electronically and your insurance will pay you directly. If you choose, or it is required by your insurance company, you can also submit your claim online.We will provide you with the necessary paperwork. 

2) We can accept assignment. You are required to pay in full, any percentage not covered by your insurance. PURE Dental Hygiene Care will accept the balance remaining directly from your insurance company. 

Once your insurance company has paid, there may be an additional outstanding balance, which is due and payable by you. If your insurance company denies any part of your claim this shall be your responsibility.
