Chronic Care Management (CCM) and Behavioral Health Integration (BHI) and Remote Patient Monitoring (RPM)

All patients who have provided consent for CCM, BHI and RPM services will be billed to the primary and secondary insurance companies. These services are billed in those months that time is spent providing those services outside of regularly scheduled visits. BNWS is aware that some insurances do not reimburse for these services as a covered benefit. In the event that an insurance company does not pay as the primary or secondary insurer, the patient is responsible for any balances that are due. The BNWS Financial policy on page two (2) requires that a credit card be kept on file to cover any balances that may be due.

By signing and dating this document, I acknowledge that I have read and agreed to the BNWS policy described above.
