• Payment Policy Acceptance

  • Thank you for choosing Dr. Elliott Kronenfeld. I am committed to providing you with exceptional care and service, as well as making my billing processes as simple and efficient as possible.

    Recent shifts in the healthcare industry have resulted in insurance companies increasingly transferring costs to my clients/patients, you, the insured. Additionally, new financial regulations put in place to protect the consumer have been implemented by the Federal and State government. This is driving many practices to adopt new financial policies to enable more efficient operational processes. Some insurance plans require deductibles and co-payments in amounts not known to you or us at the time of your visit.

  • At this time, the only insurance accepted by Dr. Kronenfeld is the Blue Cross family of plans (to include Anthem). If you are unsure whether your plan covers your appointment, please call the number on the back of your insurance card, or discuss with Dr. Kronenfeld.

    For claims being processed through accepted insurance, you will typically receive the EOB before I do, so if you disagree with the patient responsibility balance owed, it is your responsibility to contact your insurance carrier immediately. If you have out-of-network benefits for another insurance plan, Dr. Kronenfeld will collect the full fee for service and offer a "superbill statement" quarterly so that you can apply for reimbursement with your insurance plan.

  • To streamline the billing and payment system and to provide a seamless, convenient way for clients/patients to pay their bills, effective July 1, 2022, I require all clients/patients to make payments by cash, check, venmo, or if in person, credit/debit card or HSA. All known charges are due at the time of service. Charges include any copay, deductible, out of pocket expense, or missed session fee.

    Missed session fees are charged for any session that is canceled or missed without a full 24 hour notice. The full fee for all sessions is $200. If there is a missed session fee, another appointment will not be able to be scheduled until the missed session fee is paid for.

    If there is a problem with your bill/claim and it is brought to my attention after your payment is processed, I will investigate it and, if you are owed a refund, I will refund the amount in a timely manner.

  • Acknowledgment

    I understand that I am responsible for payment for all services provided to me by Dr. Kronenfeld.

    I understand that my insurance may deny or delay payment for these services or only partially pay them, or that my insurance may not cover services, and I am ultimately responsible to engage with my insurance plan to resolve issues with coverage.

    Dr. Kronenfeld submits claims to accepted insurances as a courtesy. I am still responsible for outstanding fees.
