If either the student or instructor are dissatisfied with the chair's decision, they both retain the right to request review by the CAO.
Use this form to request Level 2: CAO Review of a current Grade Grievance following the chair's decision. This request must be submitted no more than ten business following the date you were notified of the chair's decision.
The CAO will intitiate the review within five business days of receipt of this form. The CAO may form an Academic Grievance Committee, if needed, to conduct the review.
All parties will be notified of the CAO's decision, which will be submitted within five business days following the final review meeting. The decision of the CAO is final.
