• Contacts to Mpox Symptom Monitoring

    This form is to be completed daily (21 days from their date of exposure with day of exposure counting as day 0) to monitor for any signs or symptoms of mpox. Please check your temperature every day and your skin for any new rash, swollen lymph nodes, or other new symptoms.
  • Symptoms to watch for include:

    • Fever; greater than 100.4 F
    • Rash
    • Enlarged Lymph nodes
    • Cough
    • Eye Lesions
    • Conjunctivities
    • Itching
    • Vomiting or Nausea
    • Chills
    • Tired; more than normal
    • Headache
    • Urgency to defecate
    • Rectal Bleeding
    • Rectal Pain
    • Pus or Blood on Stools

    Step 1: Begin limiting contact with others and isolate yourself at home. (Isolation at home information)

    Step 2: Call your primary health care provider—notify them you were identified by the public health department as a close contact to a mpox case and explain your symptoms.
