CHECKLIST FOR WHEN TO REFER TO A SPEECH-LANGUAGE PATHOLOGIST

TO EMBRACE THE EMPLOYMENT FIRST INITIATIVE and ENHANCE ADULT LIVING
Speech-Language Pathologist are professionals that work to prevent, assess, diagnose, and treat speech, language, social, cognitive-communication, and swallowing disorders. The clinicians at Britteny Asher Consulting use this expertise to identify and eliminate potential expressive and receptive communication barriers to gaining and maintaining successful community based employment through assessment, direct services and training for employment team members, including employers.
To help identify if services would be beneficial for supporting the individual within employment, please check mark those items below which require support:

Actively engaging and participating in all aspects of the Vocational Rehabilitation process.Describing their strengths, needs and challenges.Using speech and/or their Speech Generating Device/Speech App to communicate successfully across all environments(e.g., home, community and work) and with all communication partners.Initiating new tasks or engage in interactions.Remaining aware of their environment and adhering to social and safety guidelines.Recognizing social cues and appropriately engage in social interactions.Understanding instructions and expectations.Transitioning to a new task.Requesting assistance and information when needed.Completing personal care activities within the home environment to support personal hygiene, health and safety to maintain employment.Working independently to complete steps in a newly learned task and/or new environment.Advocating for themselves to express their interests and request appropriate accommodations.Gaining information from visual, auditory and/or tactile input within the environment.¨ Using the phone, text and/or email to effectively communicate for job development/employment.Using the phone,text and/or email to effectively communicate for job development/employment.Using positive behaviors skills to promote and support positive opportunities andinteractions.Trialing and recommending Assistive Technology (AT) and/or Augmentative-Alternative Communication (AAC) to support success within the workplace.Understanding directions, organization, sequencing and /or stepsin planning.Remembering and using previousknowledge/information.
