What is Semaglutide?
Semaglutide is an injectable prescription medicine used for adults with obesity or who are overweight (excess weight); and who may also have weight-related medical problems. It is used to help adults lose weight and keep the weight off.
Semaglutide should be used with a reduced-calorie meal plan and increased physical activity.
Semaglutide should not be used with other semaglutide-containing products or other GLP-1 receptor agonist medicines.
It is not known if this medicine is safe and effective when taken with other prescription, over-the-counter, or herbal weight loss products.
It is not known if semaglutide can be used safely in people with a history of pancreatitis.
It is not known if semaglutide is safe and effective for use in children under 18 years of age.
Important information
Semaglutide may cause serious side effects, including:
Possible thyroid tumors, including cancer. Tell your healthcare provider if you get a lump or swelling in your neck, hoarseness, trouble swallowing, or shortness of breath. These may be symptoms of thyroid cancer. In studies with rodents, semaglutide and medicines that work like semaglutide caused thyroid tumors, including thyroid cancer. It is not known if semaglutide will cause thyroid tumors or a type of thyroid cancer called medullary thyroid carcinoma (MTD) in people.
Do not use this medicine if you or any of your family have ever had a type of thyroid cancer called medullary thyroid carcinoma (MTC), or if you have an endocrine system condition called Multiple Endocrine Neoplasia syndrome type 2 (MEN 2).
Who should not use semaglutide?
You should not use semaglutide if:
-you or any of your family have ever had a type of thyroid cancer called medullary thyroid carcinoma (MTC) or if you have an endocrine system condition called Multiple Endocrine Neoplasia Syndrome Type 2 (MEN 2).
-you have had a serious allergic reaction; to semaglutide or Vitamin B12. Symptoms of a serious allergic reaction include swelling of your face, lips, tongue, or throat.
-problems breathing or swallowing
-severe rash or itching fainting or feeling dizzy
-very rapid heartbeat
How should I use semaglutide:
If you miss a dose and the next scheduled dose is more than 2 days away (48 hours), take the missed dose as soon as possible.
Use semaglutide precisely as your healthcare provider directs.
You can take semaglutide with or without food.
If you take too much semaglutide, you may have severe nausea, vomiting, and low blood sugar. Call your healthcare provider or go to the nearest hospital emergency room right away if you experience any of these symptoms.
Semaglutide may cause serious side effects including:
-inflammation of your pancreas (pancreatitis). Stop using semaglutide and call your healthcare provider right away if you have severe pain in your stomach area (abdomen) that will not go away, with or without vomiting. You may feel the pain from your abdomen to your back.
-gallbladder problems, semaglutide may cause gallbladder problems including gallstones.
-kidney problems (kidney failure). In people who have kidney problems, diarrhea, nausea, and vomiting may cause a loss of fluids (dehydration) which may cause kidney problems to get worse. It is important for you to drink fluids to help reduce your chance of dehydration.
-serious allergic reactions. Stop using semaglutide and get medical help right away, if you have any symptoms of a serious allergic reaction including swelling in your face, lips, tongue, or throat, problems breathing or swallowing, severe rash or itching, fainting or feeling dizzy, or have a rapid heartbeat.
-increased heart rate, semaglutide can increase your heart rate while you are at rest. Your healthcare provider should check your heart rate while you take semaglutide. Tell your healthcare provider if you feel your heart racing or pounding in your chest and it lasts for several minutes.
-depression or thoughts of suicide. You should pay attention to any mental changes, especially sudden changes in your mood, behaviors, thoughts, or feelings. Call your healthcare provider right away if you have any mental changes that are new, worse, or worry you.
-gastroparesis. Semaglutide is a GLP-1 receptor agonist. Semaglutide is not recommended for those who experience symptoms of diabetic gastroparesis as this class of drugs can exacerbate symptoms of diabetic gastroparesis. It is recommended that you have a discussion with your healthcare provider (primary care provider and/or gastroenterologist) prior to initiating GLP-1 receptor agonist medicines.
The most common side effects may include:
-nausea
-diarrhea
-vomiting
-constipation
-stomach (abdomen) pain
-headache
-tiredness (fatigue)
Before you use your semaglutide for the first time, talk to your healthcare provider or your caregiver about how to prepare and inject semaglutide correctly.
I hereby authorize the following procedure: Sub-cutaneous administration of semaglutide.
This procedure is recommended for weight loss. I expressly represent and warrant to Presidential IV Hydration LLC that I have never been diagnosed with, nor treated for any diseases, illnesses, or conditions which may result in increased risk when I participate in regimens, programs, or services available by Presidential IV Hydration LLC and I am choosing not to participate with any expectation that Presidential IV Hydration LLC will screen for, diagnose, monitor or otherwise provide any care or treatment for such conditions.
The procedure involves a weekly subcutaneous injection and a monthly physical exam.
This procedure may be considered medically unnecessary. It may or may not mitigate, alleviate, or cure the condition for which it has been prescribed. This therapy has been recommended to you in the belief that it is of potential benefit in these circumstances and its use will probably improve the condition for which you are under treatment and your overall health.
Based on the risks and potential benefits of the current medically indicated treatment(s) and of this proposed treatment, I have elected to forego or supplement the indicated treatment(s) and receive this proposed treatment from the health professionals at Presidential IV Hydration LLC as is appropriate and necessary for my care.
I assume full liability for any adverse effects that may result from the non-negligent administration of the proposed treatment. I waive any claim in law or equity for a redress of any grievance that I may have concerning or resulting from the procedure, except as that claim pertains to the negligent administration of this procedure.
The risks involved and the possibilities of complications have been explained to me. I fully understand and confirm that the nature and purpose of the aforementioned treatment to be provided may be considered unproven by scientific testing and peer-reviewed publications and therefore may be considered medically unnecessary or not currently indicated. I acknowledge no guarantees have been made concerning the results intended from this procedure and that participating in them carries risks. I have been given an opportunity to ask questions, and all of my questions have been answered fully and to my satisfaction.
I hereby place myself under your care for Semaglutide therapy and agree to the above release. I also verify that all information presented to the medical provider in my medical history is true to the best of my knowledge. I am not misrepresenting myself and I place myself under your care for the sole purpose of treatment for these conditions.
I agree to be responsible for payment at the time of service for all services.
