  • The Health Improvement Collaborative (HIC) Community Health Survey!

  • The Health Improvement Collaborative of SECT (HIC) is a group of organizations and community members committed to working together to create opportunities for health for all our community.  The Health Improvement Collaborative has a vision of Southeastern CT as a community healthy in body and mind that promotes access, health equity, social justice, inclusiveness, and opportunities for all. Our group’s values include the intentional creation of a culture of trust, authenticity in seeking community involvement, inclusiveness, respectfulness of cultural consideration and differences, and social justice.

    The purpose of this study is to (1) inform and (2) identify and prioritize significant health needs of people living in the area. Your feedback is highly important, and it will guide our efforts to drive change.  We plan to analyze the data obtained from this survey, and then we will circle back to you and share the results with you. We appreciate the time you’re willing to take to answer the questions below.  

    This survey is intended for residents of the following cities, towns and Tribal Nations: East Lyme, Eastern Pequot Tribal Nation, Gales Ferry, Groton, Ledyard, Lyme, Mashantucket Pequot Tribal Nation, Mystic, New London, Niantic, North Stonington, Old Lyme, Quaker Hill, Stonington, and Waterford.
