• 2023 Routine Vaccination Clinic Registration

    These vaccination clinics are meant for uninsured/underinsured children 18 years of age and younger needing any routine vaccination OR those 19 years or older needing ONLY a Tetanus/Diphtheria/Pertussis (Tdap) vaccination regardless of insurance. UPDATE: we are also able to accommodate COVID-19 vaccinations for ages 6 months and up regardless of insurance as vaccine availability allows.

    CHILD (18 years old and under, uninsured/under insured):

    • Dtap/Tdap (diphtheria, tetanus, pertussis)
    • HPV (human papillomavirus)
    • Hepatitis A
    • Hepatitis B
    • Hib (haemophilus influenzae type B)
    • Meningitis/meningococcal groups A,C,W, and Y
    • Meningitis/meningococcal group B
    • Pneumococcal
    • Polio
    • Rotavirus
    • MMR (measles, mumps, rubella)
    • Varicella (Chickenpox)
    • Flu/Influenza (limited availability seasonally) 
    • COVID-19 (as supply allows, regardless of insurance)

     

    ADULT (19 years old and over, regardless of insurance):

    • Tdap (tetanus, diphtheria, pertussis)
    • COVID-19 (as supply allows)

  • De Pere Health Department does not bill insurance and can only administer state supplied vaccines to specific populations. If your child is insured and vaccines are included in the coverage, please see your Primary Care Physician (PCP) or a local pharmacy for vaccinations. If you need assistance or have additional questions, please call us at (920) 339-4054.

  • Recommended vaccines will be determined by the nurse based on availability, age, and previous vaccines received as indicated in the Wisconsin Immunization Registry (WIR). Vaccine information sheets (VIS) for each recommended vaccine will be provided at the appointment for the patient/parent to review.  

    If you/your child has received vaccines outside of Wisconsin that may not be in the Wisconsin Immunization Registry (WIR), please bring or send (deperehealth@deperewi.gov) your/your child's records to De Pere Health Dept prior to your appointment. 

    Anyone receving a COVID vaccine will be asked to wait 15-30 minutes after your appointment for monitoring based on allergy history
