1- If you commit to working with a Waiver HCSS case, please ensure that you arrive to every shift on time. If you are running late, do not contact the participant directly. Please contact the office of the Waiver On-Call Coordinator.
2- If you need to call out from a shift, please do so with adequate notice, a minimum of 4 hours prior to the start of your shift. Please call the agency’s main office or after hours on-call directly. Do not call the participant. Please be advised that if you accept a case, it is expected that you work all of the shifts consistently.
3- It is your responsibility to report all issues regarding the participant you are working with. This includes changes in behaviors, medical condition, psychiatric concerns, and environmental concerns/changes.
4- The goal of the waiver program is to promote independence, ensure health and safety, and enhance participant’s quality of life. It is our responsibility to ensure this for all participants.
5- It is expected that you treat coworkers with dignity and respect. When relieving and being relieved, it is expected that you greet and speak with your coworkers in a courteous manner. It is expected that you give a report of what occurred on the previous shift to the HCSS who is relieving you.
6- You are expected to accompany and advocate for participants on all outings into the community including medical appointments. You are expected to report any pertinent medical information to the office immediately.
7- Engage and interact with your participant(s) in a positive manner. Encourage participants to work on goals, participate in activities, and communicate/socialize.
I have reviewed the HCSS Employee Guidelines. I understand my responsibilities as an employee and I agree to carry out these responsibilities. I also understand that failure to do so may result in disciplinary action up to and including termination. My signature certifies that I agree to comply with the Agency’ guidelines for working with Waiver patients.
