  • Pfizer COVID-19 Booster

    Please note: The goal of this form is to know if you can get a booster of the Pfizer-BioNTech COVID-19 Vaccine, Bivalent (Original and Omicron BA.4/BA.5).

    To get your booster, you must have had 2 primary doses of the Pfizer or Moderna vaccine, or any booster dose at least 2 months ago. For Johnson & Johnson, you must have had at least 1 dose at least 2 months ago.

    IMPORTANT - Please Read:  If you choose an appointment date less than 2 months from your 2nd Pfizer or Moderna COVID-19 dose, or 2 months from your first Johnson & Johnson COVID-19 vaccine, you will NOT be able to receive the booster on that date.

    If you are 16 or older and moderately to severely immunocompromised, and looking to get your 3rd dose of Pfizer COVID-19 vaccine:

    1) and it has been at least 28 days since your last Pfizer COVID-19 vaccine

    2) Please call (417) 334-4544 x577 to schedule an appointment

  • Currently, due to state orders, you must be 12 or older and meet certain criteria to receive the Pfizer booster.

    If you are age 16 -17, and moderately to severely immunocompromised, please call (417) 334-4544 x577 to make an appointment.

    People with Certain Medical Conditions

  • Currently, due to Missouri DHSS orders, you must be 12 or older and meet certain criteria to receive the Pfizer COVID-19 booster vaccine.  Based on your response you do not qualify at this time.  We will update our scheduling links as DHSS orders are updated.

  • Please Note: It is important that you have your COVID-19 Vaccination Record Card so that we can verify your eligibility to receive the Pfizer COVID-19 booster vaccine.  If you do not know where your card is, please contact the facility where you were vaccinated to obtain a record of your COVID-19 Vaccination(s), then return to this page to register.

  • Please note: It is important that you bring your COVID-19 Vaccination Record Card with you to your appointment. This is so we can check if you are able to receive the Pfizer booster.
