Please fill out and submit this form to register for your preferred Foundational Family Services session. This group is for siblings of an OAP Registered individual. To be eligible for this service, the participants sibling must have a diagnosis of Autism Spectrum Disorder (ASD) and be registered in the Ontario Autism Program (OAP). Before you proceed please ensure you have your OAP Registration Number handy.
