Angels DOES NOT require the individual to work ONLY for Angels, However, as an independent contractor may choose to do so.

Angels DOES NOT require the individual to work ONLY for Angels, except, as an independent contractor may choose to do so.

Angels DOES NOT establish a quality standard for the independent contractor, except, Angels may provide plans and specifications regarding work but cannot oversee the actual work or instruct the individual as to how work will be preformed. ***This is why the plans are created from what the client asks for, what family asks for, rules and regulation definitions of services, service plan details, and the information each contractor provides during result monitoring. As the results of the service is something we can and do monitor for best practice and to remain in compliance with rules and regulations. Please, never feel this limits you on asking for mentorship. As independent contractors you have the legally autonomy but as community members, we can also ask for help and result consulting as business owners.

Angels DOES NOT pay the independent contractor a salary or hourly rate instead of a fixed or contract rate.
*

Angels DOES NOT terminate the contract provided during the contract period unless the individual violates the terms of the contract or fails to produce a result that meets the specifications of the contract.

Angels DOES NOT provide more than minimal training for the individual. Hence, why all our training is tied to rules and regulations as they dictate the minimal training we must have.

Angels DOES NOT provide tools or benefits to the independent contractor, except, that materials and equipment may be supplied.

Angels DOES NOT dictate the time of performance, except that a completion schedule and a range of agreeable work hours may be established. ***This is why you are not told when you have clients, you are asked when you have scheduled clients, so that their plans may be updated but you have control of the time of performance so long as it is meeting the client's needs.

Angels DOES NOT pay the independent contractor personally instead of making payment or checks payable tot he trade or business name of the individual
*

Angels DOES NOT combine the business operations in any way with the individual's business operations instead of maintaining all such operations desperately and distinctly.

THE INDEPENDENT CONTRACTOR UNDERSTANDS THAT THEY WILL NOT BE ENTITLED TO ANY WORKERS' COMPENSATION BENEFITS IN THE EVENT OF AN INJURY, IS OBLIGATED TO PAY ALL FEDERAL AND STATE INCOME TAX ON ALL MONEY EARNED WHILE PREFORMING SERVICES FOR THE BUSINESS, AND IF CONTRACT IS TERMINATED IS NOT ENTITLED TO IMMEDIATE RELEASE OF FUNDS.

I agree and understand
I am a volunteer and this does not apply. I understand that if as a volunteer I am hurt I have waived all liability and holds Angels Service LLC harmless and not responsibly as part of my volunteer agreement.
I understand that I am an independent contractor with Angels and not an employee.

Care Coordination and Client Initiation
As the owner I have made the executive decision to have a certain amount of flexibility to respect providers and clients alike. For some families they want to take to the main agency first, while others do better with talking to the direct provider first. Either way, before starting services we must have the permission from the case manager first and a basic intake complete in order to be in compliance. At times we also accept a soft authorization directly from the case manager rather than waiting for the formal service plan. This allows us to provider services sooner which is better for the families. This also means that when working with new clients or clients who are doing their annual review, we may see denied claims on our billing reports. I will take care of that part while you focus on your service. That is why we work together. :)
Please select all those who are part of care coordination

Client Updates
Please list each of your clients and provide one strategy so that we may add to their plan on how you meet their goals. I have provided three samples here: 1: Family will spend 20 minutes each night in a bedtime routine in order to help reduce nighttime stress and promote sleep. (service: Developmental Therapy goal: a more relaxing bedtime that is consistent and shorter). 2: George will spend ten minutes practicing his mediation before he goes to work to help keep calm on the bus. (service: mentorship goal: to be able to go to work calm in order to avoid conflict with supervisor). 3: We will follow a routine of washing our hands and singing a song that introduces touch prior to each massage session (service: massage goal: to reduce tactile defensiveness and promote acceptableness of touch). Remember, what you write goes into their plan at Angels and when we check in with case managers it is what we share with them.
If you prefer to upload your care coordination / result monitoring client review document you may. Please may sure it is editable as I will need to put it into the plans.
