My medication is not at my pharmacy
Before completing this form please make sure you meet one of these criteria:
1) You have requested a refill of your medication; if not, please complete the REFILL MY MEDICATION form
2) You have had an in-office appointment in the past 7 days
If you had an appointment in the office, your medication will be at your pharmacy on the following day. When contacting your pharmacy, have the pharmacist check your profile.
DO NOT CALL THE OFFICE as this will delay the processing of your request. You will be notified by text/ email when your Rx has been sent to your pharmacy.
