Insurance billing and Credit Card on File Policy
Stork and Cradle will verify your insurance coverage at the time of class registration. We will bill the insurance on the class date. Should your coverage change at that time, co-insurance, co-payments, and/or deductibles will be applied to your coverage. We will charge your card for those fees indicated by your insurance plan.
At Stork and Cradle, we require keeping your credit, HSA or debit card on file as a convenient method of payment for the portion of services that your insurance may not cover, but for which you are liable. A billing fee of $10 will be added to your account for any balances that we must attempt to collect through mailing monthly statements. Furthermore, an "outstanding balance" charge of 2 percent of the total bill will charge for each month that the bill remains unpaid.
Your credit card information is kept confidential and secure and payments to your card are processed only after the claim has been filed and processed by your insurer, and the insurance portion of the claim has paid and posted to the account.
You will be charged the full class fee if you are a no-show to any registered class. Cancellations of less than 1 week before the start of any class will be charged a 50% fee.

I understand my card will not be charged at this time. My card will be stored securely on Stripe's payment portal and not on any other servers to be possibly charged at a later date as stated in this policy.
