IF YOU DO NOT RECEIVE A CONFIRMATION EMAIL, IT IS YOUR RESPONSIBILITY TO CALL 531-355-5039 TO CHECK ON THE RECEIPTOF YOUR FORM. FAILURE TO CHECK THE STATUS IF NO CONFIRMATION IS RECEIVED WILLIMPACT YOUR CLAIM IF THE 7 DAY DEADLINE PASSES.
Grievance claims;submitted after the 7-day deadline or if the results of the test have already been received (refer to number 4 under EIPA Test Grievance Policy) will not be accepted. During the investigation process, you may be contacted if additional information is needed. In all cases, the decision as to whether the circumstance(s) qualify as a grievance event will be at the sole discretion of the EIPA Diagnosis Center.
