Self Enhancement, Inc, Immunize Oregon, Legacy Health, Multnomah County, and REACH are excited to host this community vaccination clinic focused on serving Black, Afro-Caribbean, African Immigrants and Refugees and other people of color eligible to receive the COVID-19 vaccine in Oregon. Individuals ages 16 years and older are qualified to receive the Pfizer COVID-19 Vaccine during this event.

Entrance will be on Shaver St. near Kerby Ave. The neighborhood provides enough street parking to accommodate this event, but please arrive no more than 15 minutes early for your appointment to reduce parking needs. For individuals with limited mobility, we have a designated drop-off zone with wheelchair accommodations. These clinics can take an hour or more from start to finish so please plan accordingly.

Safety is our number one priority, which is why we want to make sure we have as much patient information before you arrive to your appointment. Providing this information ahead of time will ensure a smooth check-in process and get you through the line faster! If you are an intended participant for this vaccination event, please complete this form and select the date and time that works best for you. Once your appointment is scheduled, you will receive an email confirmation from Immunize Oregon. Each individual will need to schedule an appointment to receive their COVID-19 vaccination at this event.
