• You will need to complete the following health survey, for your Macintyre Health consultant, to the best of your ability.
• General Billing Information: It is important to note that Macintyre Health is not a bulk billing service. Our fees are transparently listed on our website at www.macintyrehealth.com.au. We encourage our patients to visit our website to familiarise themselves with our fee structure.
• Bulk Billing Policy: At Macintyre Health, we are committed to providing accessible healthcare services to our community. As part of this commitment, we proudly offer bulk billing for patients under 21 years of age for all diabetes-related appointments, and may bulk bill otherwise at our discretion.
• Payment Policy: Payment, for all other appointments is required in full at the time of your appointment. We accept various forms of payment, and our friendly administrative staff are here to assist you with any billing or payment queries you may have.
• Macintyre Health provides a safe environment for both patients and staff. We have a Right to Respect Policy and reserve the right to end any consult early and discharge the patient if this policy is violated.
• Any medical technology device (such as continuous glucose monitors or insulin pumps) that are delivered to your home, must not be applied until you are fully trained and in the virtual presence of a Macintyre Health clinical consultant. Doing so is against Macintyre Health's clinical advice.
• You understand that Macintyre Health provides consults by appointment only, and does not provide emergency/urgent health services.

Privacy and Consent:

Macintyre Health Australasia needs to collect information about you for the primary purpose of providing a quality service to you. To thoroughly assess, diagnose and provide therapy, we need to collect some personal information from you. If you do not provide this information; we may be unable to treat you. This information will also be used for:

• The administrative purpose of running the practice;
• Billing either directly or through an insurer or compensation agency;
• Use within the practice if discussing or passing your case to another practitioner within the practice for your ongoing management;
• Disclosure of information to your doctors, other health professionals or to teachers to facilitate communication and the best possible care for you; and
• In the case of insurance or compensation claim it may be necessary to disclose and/or collect information that concerns your return to work to an insurer or your employer

Macintyre Health Australasia has a Privacy Policy that is available on request. That policy provides guidelines on the collection, use, disclosure and security of your information. The Privacy Policy contains information on how you may request access to, and correction of, your personal information and how you may complain about a breach of your privacy and how we will deal with such a complaint.

To ensure the process of quality treatment provision, information about your assessment results and progress may be given or transferred to relevant other service providers, who are involved in your management. These may include your doctor, teachers, specialists, insurers, solicitors or employers.

I have read the above information and understand the reasons for the collection of my personal information and the ways in which the information may be used and disclosed and I agree to that use,disclosure and transfer of information.

I understand that it is my choice as to what information I provide and that with holding or falsifying information might act against the best interests of my assessment and therapy progress.

I am aware that I can access my personal and treatment information on request and if necessary, correct information that I believe to be inaccurate. I understand that if, in exceptional circumstances, access is denied for legitimate purposes, that the reasons for this and possible remedies will be made available to me.

I understand that I have an opportunity to obtain a copy of Macintyre Health Australasia's privacy policy.
